9-1-1 HONOR AWARDS NOMINATION

   E9-1-1 Institute - 5th Annual Awards Ceremony

   The E9-1-1 Institute is sponsoring its 5th Annual Honor Awards Ceremony where
   heroes and leaders in the 9-1-1 field will be honored jointly by the Institute
   and the Congressional E9-1-1 Caucus on March 4, 2008.

Nomination Form and Instructions

Instructions:

1. Nomination forms are due no later than Friday, February 1, 2008.

2. These awards will honor individuals or groups of individuals who have shown dedication
and leadership that relates to the mission of the E9-1-1 Institute, which:

Serves as an information clearinghouse on E9-1-1 and emergency communications issues and distributes timely, relevant information to Members of Congress, Administration officials, federal, state, and local public safety officials, and the general public;
Creates a forum for policy discussions related to E9-1-1 and emergency communications that will benefit public policy development; and
Assists members of the Congressional E9-1-1 Caucus in promoting public education on E9-1-1 issues.

3. Actions that are eligible are actions that took place during calendar year 2007.

4. In addition to this form, you are welcome to submit additional information, such as:

Citizen In Action Award 9-1-1 Call Taker/ Dispatcher Award First Responder Award Government Leader Award (Federal) Government Leader Award (State/Local) Industry Professional Award 9-1-1 Technician Award Outstanding 9-1-1 Call Center Award	The Citizen in Action award honors someone who has demonstrated Exceptional Bravery and Heroism to save a life in a critical time use 9-1-1. This award honors the "First First Responders" - the 9-1-1 Call-Takers and Dispatchers who are on the front line everyday. Candidates for this honor should have a proven track record of service in the field and a reputation for going the extra mile on behalf of emergency callers. The First Responder Award recognizes the individuals that 9-1-1 people call - e.g. *Law Enforcement, Fire-Rescue* and/or Emergency Medical Providers --for their professionalism, initiative, creativity or diligence in assisting a person reporting and/or experiencing an emergency situation. This award recognizes the Federal Government Leader that has done the most to advance the capabilities of 9-1-1 emergency communications. This award recognizes the State or Local Government Leader that has done the most to advance the capabilities of 9-1-1 emergency communications. The Industry Professional Award honors an individual or organization in the commercial sector that has demonstrated exceptional contributions toward the advancement of emergency 9-1-1 communications. This award recognizes the Technology Support Staff - e.g. Information Technology, GIS and other technical personnel with a 9-1-1 authority or jurisdiction whose tireless efforts make available and maintain the critical technology systems necessary to answer, process and respond to emergency calls. This award recognizes a *state, regional or local 9-1-1 program* for its excellence in key emergency communications areas ranging from the handling of emergency situations reported to 9-1-1 to the deployment and management of new technology.
